Tattoo Request Form 

Submit a request to be tattooed below. I do my very best to respond to every request in a timely manner; please be patient. If you do not hear back within 7-10 days, please try resubmitting your request. 

Due to the large number of inquiries, please be aware that unfortunately I cannot accept all new requests. I apologize for this inconvenience and would be happy to recommend you to another artist.

I do not tattoo under the age of eighteen. 

**Note: Please be thorough and include as much information as you can regarding design including placement, size, style, color or black and grey, any scars or whether this is a coverup or rework request. Please include how flexible you are to design changes and artistic interpretation. 

**Note: Please include the best days of the week for you to get tattooed. I generally work Monday through Friday, starting at 10:00am. In person consultations can be requested for 9:30am or 4:30pm weekdays.
